It could be muscle strain from intense workouts, a minor infection in the area, or even digestive issues like constipation causing referred pain. In rare cases, it could signal something more serious like an umbilical hernia (especially if you feel a bulge).

When should you worry? If the pain is severe, accompanied by fever, nausea, vomiting, or significant swelling and redness, see a doctor. They can properly diagnose the cause and recommend the best course of treatment. In the meantime, keep the area clean and dry, and avoid tight clothing to give it a break.
